Patient Safety Bill of Rights
1. Safety
Right to safe medical care of the highest standard.
2. Respect
Right to be treated with respect, with regard for personal dignity, and without discrimination as to age, ethnicity, race, gender, sexual identity, social status, economic status, disability, or medical condition.
3. Competence
Right to be treated by healthcare personnel who are trained to be safe, person-centered, equitable, and effective in the area in which they are practicing.
4. Environment
Right to a healthcare environment that is clean and safe for you and for the persons providing your care.
5. Information
Right for timely, honest and complete information of your healthcare status and the plan for your care, in language that you can understand, including the name, purpose, risks, and benefits of the the treatments and medications you are offered.
6. Access to records
Right to access your medical record at any time.
7. Shared Decision Making
Right to make your own healthcare decisions, including the decision to decline recommended care.
8. Support
Right to have a family member or supporter present with you when receiving healthcare and to name a person to speak for you at times you cannot.
9. Credentials
Right to know the names and credentials of the persons providing your care.
10. Full disclosure
Right to have your opinion heard on the safety of your care and to know if things go wrong.
