“Soft signs” that a patient is not doing well

You do not always need formal measurements like heart rate, blood pressure, or breathing rate to know when a patient is seriously ill. Nurses also use “soft signs” in appearance and behavior to assess a patient’s condition. Friends and family members can use some of the same signs to decide if a patient needs immediate help.
